Derrick Wayne Kuokoa Cook of Seagrove Beach, Florida passed away on Tuesday, November 3, 2015 at Sacred Heart Hospital.  He was 40.

Memorial services will be at 11:00 a.m. Thursday, November 5, 2015 in the chapel of Sorrells Funeral Home & Crematory in Enterprise with Pastor Steve Warren officiating.  The family will receive friends Thursday beginning at 10:00 and continuing until service time.

Flowers will be accepted, or donations may be made to National Kidney Foundation, Finance Dept, 30 East 33^rd St., New York, NY 10016.

Derrick was born June 13, 1975 in Honolulu, Hawaii.  He was a children’s and family therapist, and he also served as a Captain in the United States Army Reserves.

He was preceded in death by his father, Charles Wayne Cook.

Survivors include his wife, Mandy J. Cook of Santa Rosa Beach, FL; his mother, Ann Dora Cook of Enterprise, and two daughters, Avery Taylor Cook and Grayson Warner Cook, both of Enterprise.  He also leaves behind many aunts, uncles, nieces, nephews, and friends who loved him very much.
